Two switches on a waiver template decide how much weight it carries. Required for diving makes it part of every waiver check, and Medical waiver adds the diver medical questionnaire to the signing page.
Required for diving
Set Required for diving to Required in the template dialog. Only active templates count. A required template:
- shows the Required for diving badge in Settings;
- decides the Waivers word in the customer header;
- appears in each dive group's Waiver check and in the trip's departure check, except for course groups;
- feeds the dashboard's Missing waivers panel;
- triggers the Waiver expires before trip notification, for online signatures only.

Note: Required means flagged, not enforced. DivePilot never blocks a booking, a place in a dive group or a seat on the boat because a waiver is missing. Checking before departure is up to your team.
A required template is not sent automatically either: nothing goes out when a customer books online or registers. You request each signature yourself.
Medical waiver
Switch Medical waiver to Is a medical questionnaire for a medical statement, such as the agencies' medical forms. When a customer opens the signing link, a Medical questionnaire with ten questions appears above the name and signature. Each is answered with Yes or No. They ask about:
- lungs, breathing, heart or blood problems;
- being over 45;
- struggling with moderate exercise;
- eye, ear or sinus problems;
- surgery in the last 12 months;
- loss of consciousness, migraines, seizures or head injury;
- psychological treatment or addiction;
- back problems, hernia, ulcers or diabetes;
- stomach or intestine problems;
- prescription medication.
Every question must be answered before Sign the waiver can be pressed. The questions are the same for every medical template and every agency, and you cannot edit them or add your own. They appear in the language the signing page is shown in.
When a diver answers yes
Any Yes stops the online signature. The diver sees Medical evaluation required, which tells them to contact the dive center and bring a signed medical clearance. The name, signature and sign button are greyed out.
What this means for you:
- Nothing is saved and nobody at the dive center is notified. The request shows Link sent until the link expires after 48 hours, then Unsigned. If the template has a validity period, the request shows Expired as soon as that period, counted from the request, has passed.
- The diver can change the answer back to No and sign. The change leaves no trace.
- When the diver brings the physician's form, use Upload signed copy or Photograph signed copy on the customer's Waivers tab. These are not offered on a record whose link is still active, so press Request waiver again. See Request a waiver signature.
A medical waiver signed online lists the answers under Medical answers on the record. Since any Yes stops the signature, stored answers are always No. An uploaded or in-person medical waiver has no stored answers; the proof is the paper form.
Who can see the answers
Anyone with the permission to view waivers can open Documents and every customer's Waivers tab. That shows the medical answers, the drawn signature, the IP address and the device, and opens uploaded copies such as a physician's form. Every built-in preset includes this permission, Divemaster, Boat master and Instructor too. It has no "own" version, so Documents lists all customers, not only those on a person's own trips. There is no separate permission for medical data.
To narrow it down:
- Untick "waivers view" in the Waivers group of that person's permissions. Presets only prefill, so changing a preset alone changes nobody's access.
- This also hides Documents, the customer's Waivers tab and header word, and the waiver notifications, so the person can no longer request waivers either. The trip's Check waivers keeps working, as it needs no waiver permission.
- For people you add later, create a preset without it. See Roles & presets.
