Some waivers include a diver medical questionnaire. It appears on the signing page above your name and signature, with ten questions that each need a Yes or a No.
The ten questions
- Have you had problems with your lungs, breathing, heart, or blood affecting your normal physical or mental performance?
- Are you over 45 years of age?
- Do you struggle to perform moderate exercise (e.g. walk 1.6 km / 1 mile in 14 minutes or swim 200 m without resting), or have you been unable to participate in normal physical activity due to fitness or health reasons within the past 12 months?
- Have you had problems with your eyes, ears, or nasal passages/sinuses?
- Have you had surgery within the last 12 months, or do you have ongoing problems related to past surgery?
- Have you lost consciousness, had migraine headaches, seizures, stroke, significant head injury, or do you suffer from persistent neurologic injury or disease?
- Are you currently undergoing treatment (or have required treatment within the last five years) for psychological problems, personality disorder, panic attacks, or an addiction to drugs or alcohol, or have you been diagnosed with a learning or developmental disability?
- Have you had back problems, hernia, ulcers, or diabetes?
- Have you had stomach or intestine problems, including recent diarrhea?
- Are you taking prescription medications (except birth control or anti-malarial drugs other than mefloquine/Lariam)?
While a question is still unanswered and none is answered Yes, the page says "Answer all the medical questions to enable signing." under the sign button.
If you answer Yes
A single Yes, to any of the ten questions, means this form cannot be signed online. The page shows "Medical evaluation required" and explains that a physician must assess your fitness to dive first. Your name, the signature box, the two checkboxes and the sign button are greyed out.
This applies to every question, including your age: if you are over 45, you cannot complete this questionnaire online.
What to do next
- Contact the dive center and tell them you answered Yes. The page does not tell them: when the form is blocked, nothing is saved.
- Ask the dive center which form your doctor should sign, then see a doctor.
- Bring the signed medical clearance to the dive center. They add it to your record themselves, so you do not need the online link again.
Leave enough time before your trip, since a doctor's appointment may take a while to arrange.
Answer honestly
Nothing stops you from changing an answer back to No, but the questions exist for your safety underwater. If a question is unclear, ask the dive center or a doctor before you answer. Your answers are stored with your signature, and only the dive center can see them.
The language of the questions
The questions follow the page language, which you can change with the language button at the top of the page. The waiver document next to them stays in the language the dive center uploaded. See Pages and emails in your language.
