Booking online and registering with a dive center use the same form. A card on the first step estimates "About 3 minutes" and reminds you that your details go straight to the dive center.
Moving through the steps
The form is split into short steps, shown as pills under a line such as "Step 1 of 4" and a progress bar. Press Next and Back to move, or tap a pill to jump to that step. Required fields carry a star; the others say "(optional)". The dive center decides which fields appear, so your form may have fewer steps.
Basic info
First name, Last name, Email and Language are always required. The dive center may also ask for Phone, Date of birth, Sex and Nationality.
Check your email address carefully: booking emails, waiver requests and dive logs all go there. Language sets the language of those emails. It starts with the language of the page, so change it if you prefer another one.
The email address cannot be left out. If you have none, ask the dive center to take your details at the shop instead. For a child or partner without their own address, you can enter yours: the emails about bookings made online for them, and their waiver requests, then arrive in your inbox, addressed to them by name. If you dive together in the same group, the dive log email arrives only once and greets just one of you, but its link works for everyone.
Address and experience
Address asks where you live. Experience asks for your Total dives and Last dive date, and lets you add your certifications. Press Add cert, choose the Certification level, and add the Cert number and Issued at date if you have them. Add one card per certification. The level list holds the agencies and levels the dive center works with. If yours is not there, press the bin icon on that card and bring your certification card instead.
Health notes
This step can ask for your Passport number, Dietary requirements, Medical notes and the Weight belt you normally use, in kilograms. When the gear list has its own weight belt row, you enter the kilos there instead. It is called Health when you book and Health and gear when you register.
These notes are not the diver medical questionnaire. If the dive center needs that, it is part of a waiver they ask you to sign: see The medical questionnaire.
Rental gear
When you book, gear is its own step, Rental equipment, and appears only if the dive center rents gear online. When you register, it is part of Health and gear. Either way the question is Do you need rental equipment?

- I need some gear shows one row per item, such as a wetsuit or fins. Choose Bringing my own, Need to rent or Not needed, and pick a Size where one is asked.
When the dive center rents gear online, the list also has a Special wishes box for anything else. If the dive center shows prices, you may also see Kits, several items rented together, and once you pick gear to rent, a Rental gear total appears with "Estimated total - confirmed at pickup.". When you book, the gear you ask to rent reaches the dive center as a request on your booking; no gear is reserved for you yet. When you register, nothing is requested: for the usual items, such as a wetsuit, BCD, fins, mask, regulator, dive computer or weight belt, your choices are saved with your details as the gear you own and your sizes.
When you book, Add extras below the list lets you ask for something the rows do not cover, such as a second item of the same kind or a kit. Press Add request, choose from Equipment or kit, pick a Size or the Kit sizes where asked, and set the quantity, up to 10 per line. The form takes up to 20 request lines in all, counting each item or kit you rent from the list above; with more, sending the form shows "Too many selected - the limit is 20". Extras also reach the dive center only as requests.
Terms and sending
On the last step, tick I agree to the terms and conditions, then press Reserve my spot when booking or Submit when registering.
Messages on the form
A field shows a message under it once you leave it, or when you send the form:

If something still needs fixing when you send the form, "Fix the highlighted fields before submitting" appears and the form jumps to that step. Messages like this one pop up at the bottom of the screen and disappear after a few seconds, so press the button again if you missed one.
If you leave halfway
In the same browser, the form remembers your progress for 4 hours after your last change. When you come back, a bar says "Continuing where you left off". Your step, your trip or course (if it is still available), your gear choices and your Special wishes are kept. Everything else you typed, such as your name, contact details, address, experience, certifications and health notes, is deliberately not kept, so type it in again. The date and time of a fun dive request are not kept either. Start over empties the form, clears your gear choices and goes back to the first step. When booking, your trip or course stays picked; press Change on Your booking to pick another.
A private browsing window forgets your progress once you close it, and a browser that blocks site data remembers nothing.
Note: On a shared tablet at the dive center, if the "Continuing where you left off" bar appears, press Start over before you begin, so you do not carry on with the previous guest's gear choices. Their trip or course stays picked, so check Your booking too.
